The Details

We are looking for a driven and knowledgeable Clinical Supervisor to join our incredible team in the San Francisco area of California! As a Clinical Supervisor, you will play a vital role in supporting the DPCS and help drive the success of our clinical team. In this role, you will be responsible for guiding the clinical team on the proper processes as they care for our patients.

Responsibilities

  • Supervises Home Health nurses, LVNs, and other patient care team members
  • Ensures that high-quality patient care is delivered and that care plan goals are met
  • Reviews documentation, patient satisfaction reports, and care plans
  • Participates in and oversees team conferences conducting/participating in multidisciplinary team conferences, and reviewing patient satisfaction reports
  • Solves patient/family member concerns
  • Performs disciplinary actions and employee training; conducts performance assessments for employees
  • Ensures that company policies and procedures are followed
  • Conducts field RN visits to patients as needed
  • Conducts supervisory field visits with Home health patient care providers

Qualifications

  • Current CA RN license
  • Previous leadership/supervisory experience (preferred)
  • Experience with clinical complexities
  • Multitasking and time management skills
  • Excellent communication skills, both verbal and written
  • Exceptional interpersonal skills
  • Team-oriented
