About the Role
The Care Team Hospice is seeking an experienced and relationship-focused Hospice Clinical Liaison (RN/LPN) to help strengthen our partnerships with hospitals, skilled nursing facilities, senior living communities, physicians, and other healthcare providers. This is a unique opportunity for a clinician who enjoys being out in the community, building relationships, educating others, and connecting patients with the right care at the right time.
As a Hospice Clinical Liaison, you'll serve as the bridge between our clinical and sales teams, bringing your healthcare expertise directly to our referral partners. You'll maintain a consistent presence within assigned facilities, provide hospice education and clinical consultation, identify patients who may benefit from hospice services, and help ensure a smooth transition from referral through admission.
We're looking for someone who is clinically confident, highly personable, and self-driven. The ideal candidate can walk into a facility, build credibility with nurses and leadership, recognize opportunities to help patients, and become a trusted resource our partners know they can rely on.
Responsibilities
- Build and maintain strong relationships with hospitals, physicians, skilled nursing facilities, assisted living communities, and other healthcare referral partners.
- Serve as a clinical hospice resource by providing education, consultation, and guidance on hospice eligibility, disease progression, symptom management, and the Medicare Hospice Benefit.
- Maintain a consistent presence within assigned facilities, identify patients who may benefit from hospice services, and support timely referral and admission coordination.
- Partner closely with Community Care Liaisons, clinical leadership, admissions, and operations to remove barriers, strengthen accounts, and improve referral-to-admission conversion.
- Conduct clinical screenings and hospice eligibility assessments within RN scope of practice while ensuring appropriate communication and documentation throughout the referral process.
Requirements
- Active and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required.
- Two or more years of hospice, palliative care, home health, skilled nursing, geriatric care, or related healthcare experience preferred.
- Strong knowledge of hospice care, eligibility criteria, and the Medicare Hospice Benefit.
- Excellent communication, presentation, relationship-building, and customer service skills.
- Prior healthcare liaison, admissions, business development, or sales support experience preferred.
- Ability to work independently, manage an assigned territory, and maintain a strong presence with referral partners.
- Valid driver's license, reliable transportation, and ability to travel throughout the assigned territory required.
Travel: Ability to perform regular travel within assigned geography and extended geography for training and meetings.
Transportation: Must have reliable transportation. Valid and current driver’s license and current auto insurance required.
