Clinical Liaison- FT Days | Humble Rehab - Willowbrook, Spring and NW Houston

About the role

Are you passionate about your career, results‑driven, and love helping people? As a Clinical Navigator at PAM Health, you will be the face of the hospital, meeting patients during their most challenging times and sharing the potential success they can achieve during their rehab stay.

Responsibilities

• Cultivate referral relationships and manage an assigned territory.
• Complete patient assessments and make sales contacts.
• Analyze referral and admission data from the assigned territory.
• Collaborate with business development team members, admissions staff, nursing, and other internal and external personnel to facilitate referral conversion.
• Provide feedback and recommendations for program development and quality‑improvement initiatives related to customer service, referral and admission processes, and patient and referral source satisfaction.
• Develop and maintain excellent relationships with prospective patients, family members, physicians, ICU and floor nurses, discharge planners, case managers, and payer representatives.

Qualifications

• Degree in a health‑related field from an accredited college or university (preferred).
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech‑language pathology (required).
• Prior marketing and/or rehabilitation/LTACH experience (preferred).
• Minimum of two years’ experience in the designated licensed field, preferably with rehabilitation/LTACH and marketing experience.
