Clinical Liaison (CL) PRN, Acute Rehabilitation

About the Role

As a Clinical Liaison at St. Joseph Regional Medical Center, you will play a vital role in building and maintaining referral relationships to drive appropriate admissions to the acute rehab unit. You will assist in coordinating the intake and pre-admission screening process, perform on-site clinical assessments, and communicate rehabilitation options to patients and families.

Responsibilities

• Build and maintain referral relationships to drive appropriate admissions to the acute rehab unit.
• Assist and coordinate the intake and pre-admission screening process.
• Perform on-site clinical judgment to determine patient appropriateness for admission (typically in a referring facility).
• Communicate rehabilitation and facility options to patients and families.
• Assess patient expectations and requirements for participation.
• Secure information relating to patients' resources and benefits.
• Serve as the initial contact for external case management and discharge planner personnel with payors and hospital facilities.

Requirements

• Clinical licensure and a bachelor's degree.
• Previous clinical experience with demonstrated skills in clinical assessment and EMR systems.
• Previous marketing/sales experience preferred.
• Formal sales training preferred.
• Valid driver's license.
