About the role

We are seeking a Clinical Inpatient Rehab Liaison PRN to join HCA Florida JFK Hospital. This position offers a unique opportunity to contribute to the well-being of patients and the community.

Responsibilities

  • Assist and coordinate intake and pre-admission screening processes.
  • Document evidence of rehab admission criteria within established timeframe.
  • Perform on-site and off-site clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.
  • Provide information to patient/family/caregiver related to inpatient rehab and program features.
  • Communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.
  • Obtain information relating to patient resources and healthcare insurance benefits.
  • Demonstrate clinical knowledge of rehab admission criteria to appropriately identify and qualify rehab patients.

Requirements

  • Graduate of an accredited School of Nursing or Physical Therapy School.
  • Current State of Florida Registered RN License, or Recognized Compact Licensure (subject to Florida State Licensing Requirements, including/not limited to ongoing eligibility and duration provisions).
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • Speech Therapist, or Occupational Therapy (OT), or Registered Nurse, or Licensed Physical Therapist.
  • Masters Degree, or Associate Degree (5 years of experience preferred).

  • Ability to effectively communicate with patients, families, and caregivers.
  • Strong organizational skills to manage multiple tasks and deadlines.
  • Excellent interpersonal and problem-solving skills.
