Clinical Inpatient/Outpatient Dietitian

Key Responsibilities

• Identifies and assesses patient nutritional needs and creates care plans to address any identified risks.
• This may include but is not limited to working with critical care patients in the ICU, writing nutrition-related orders, providing appropriate nutrition education, assessing wound-related nutrition needs, and completing timely and thorough documentation in the electronic medical record.
• Assesses for and diagnoses malnutrition.
• Educates on diet according to disease state, medical condition, and health risk.
• Coordinates discharge plans for patients returning home with nutritional support needs. This includes, but is not limited to, writing orders for home nutrition support equipment, creating and educating patients on nutrition support plans, navigating insurance plans for coverage of home nutrition support, and coordinating needs with the selected home medical supply company of patient’s choice.
• Acts as consultants to rural Essentia Health site providers for high acuity nutrition care concerns.

Required Qualifications

• Upon completion of 90 days of employment, must apply for Advanced Practitioner Credentials through the Medical Executive Committee.
