About the Role

This is a PRN (as-needed) registered nurse position focused on hospice care. The role involves planning, organizing, and directing hospice services, with an emphasis on community health education and meeting the needs of patients and families in their homes and communities.

Responsibilities

  • Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs, including a full physical assessment and illness history.
  • Provides professional nursing care using all elements of the nursing process.
  • Assesses and evaluates patient status, writes and initiates the plan of care, and regularly re-evaluates patient and family needs.
  • Participates in revising the plan of care as necessary and uses health assessment data to determine nursing diagnoses.
  • Develops a care plan with goals based on nursing diagnosis, incorporating the patient and family in the planning process.
  • Initiates preventive and rehabilitative nursing procedures, administers medications and treatments as prescribed by the physician.
  • Counsels patients and families on nursing and related needs, provides health care instructions, and assists with activities of daily living.
  • Communicates with physicians regarding patient needs, reports changes in condition, and obtains/receives physician orders as required.
  • Works with the family/caregiver and hospice interdisciplinary group to meet emotional needs of the patient and family.
  • Attends interdisciplinary group meetings and supervises hospice aides.

Requirements

  • Graduate of an accredited school of nursing.
  • One to two years of recent acute care experience in an institutional setting and one to two years of recent home care experience.
  • Current licensure in the state and CPR certification.
  • Bachelor’s degree preferred; one year of hospice care experience preferred.
  • Excellent observation, verbal and written communication, problem-solving, and basic math skills.
  • Proficient in nursing skills per competency checklist.
  • Ability to engage in prolonged or considerable walking or standing, lift/position/transfer patients, and lift supplies and equipment.
  • Considerable reaching, stooping, bending, kneeling, and/or crouching.
  • Visual acuity and hearing sufficient to perform required nursing skills.
  • Must be a licensed driver with an insured automobile in good working order.
  • Hospice experience strongly preferred.

Schedule

Part-time, PRN (as-needed) position with work location primarily on the road.
