About the role
The registered nurse plans, organizes, and directs hospice care, building from community resources to meet the needs of individuals and families within their homes and communities.
Responsibilities
- Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs, including a full physical assessment and illness history.
- Provides professional nursing care using all elements of the nursing process.
- Assesses and evaluates patient status, writing and initiating the plan of care and regularly re-evaluating needs.
- Participates in revising the plan of care as necessary and uses health assessment data to determine nursing diagnosis.
- Develops a care plan with goals based on nursing diagnosis, incorporating the patient and family in the planning process.
- Initiates preventive and rehabilitative nursing procedures, administers medications and treatments as prescribed by the physician.
- Counsels the patient and family on nursing and related needs, providing health care instructions as appropriate.
- Assists with activities of daily living and facilitates patient efforts toward self-sufficiency and comfort care.
- Communicates with the physician regarding patient needs, reports changes in condition, and obtains physician orders as required.
- Works with the family/caregiver and hospice interdisciplinary group to meet emotional needs of the patient and family.
- Attends interdisciplinary group meetings.
Requirements
- Graduate of an accredited school of nursing.
- One to two years of recent acute care experience in an institutional setting.
- One to two years of recent experience with home hospice care.
- Current licensure in State and CPR certification.
- Bachelor’s degree preferred, with one year of hospice care experience.
- Excellent observation, verbal and written communication, problem-solving, and basic math skills.
- Nursing skills per competency checklist.
- Prolonged or considerable walking or standing; ability to lift, position, and transfer patients; lift supplies and equipment.
- Considerable reaching, stooping, bending, kneeling, and crouching.
- Visual acuity and hearing to perform required nursing skills.
- Must be a licensed driver with an insured automobile in good working order, meeting state/organization requirements.
