About the Role

As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. This hybrid PRN position involves in-person preadmission evaluations and assessments throughout Topeka or Wichita and its surrounding areas, combining remote work with local travel as needed.

Travel expenses—including mileage and parking—are covered and reimbursed by Acentra Health. We will provide all necessary technology equipment for your home office, including a laptop for use in the field.

Responsibilities

• Conduct PASRR (Preadmission Screening and Resident Review) assessments and evaluations, developing informed level-of-care recommendations across nursing homes, hospitals, and community environments.
• Determine the appropriate next level of care for individuals who may require long-term care placement while assessing each individual’s unique needs.
• Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.
• Ensure compliance with contractual and regulatory standards while developing level-of-care recommendations.
• Utilize independent judgment, clinical expertise, communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.
• Collaborate with healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
• Stay informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics.
• Leverage technology by utilizing the state's electronic system and proprietary platforms to manage and track assessment activities.
• Execute tasks with strong attention to detail, ensuring adherence to established workflows to maintain high standards of quality and operational efficiency.
• Read, understand, and adhere to all corporate policies, including those related to HIPAA and its Privacy and Security Rules.

Requirements

• Active, unrestricted Kansas license in one of the following: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Psychologist (LCP), Registered Nurse (RN), or another qualified Licensed Mental Health Professional (LMHP).
• Minimum of 2 years of experience assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.
• Minimum of 2 years of experience assessing and analyzing psychological, emotional, behavioral, and physical functioning, with basic knowledge of standards and best practices for determining Level of Care within mental health, geriatrics, and conditions commonly requiring long-term care placement.

Preferred Qualifications

• 2 years of experience working with individuals with Intellectual Disabilities and/or other related conditions.
• 1 year of experience conducting Preadmission Screening and Resident Review (PASRR) requirements, including applicable federal regulations and state-specific rules.
• Strong verbal, written, and electronic communication skills with the ability to engage effectively with individuals and groups.
• Detail-oriented with strong organizational skills and the ability to prioritize and complete tasks efficiently within established timelines.
• Familiarity with public mental health and medical programs.
• Previous experience working with the geriatric population.
• Proficiency in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).

Pay

The compensation for this role is $36.00 to $40.00 per hour. An applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
