Clinical Diabetes Nurse

About the role

The Clinical Diabetes Nurse will be an integral part of the team, working both behind the scenes and in the clinic. This role involves collaborating with providers and clinical staff to ensure high-quality, timely care is provided, using the nursing process to plan, direct, provide, and evaluate patient care in collaboration with the healthcare team.

Responsibilities

• Greet visitors and clients in a friendly and professional manner.
• Triage and develop a plan of care, determining patient needs.
• Coordinate care and pre-visit planning.
• Collaborate with an interdisciplinary team to evaluate clinical care or health services.
• Conduct monthly multidisciplinary team meetings focused on diabetes care.
• Document medical records with organized and high-quality entries as required.
• Work collaboratively with providers to deliver high-quality care.
• Assist the Endocrine Specialist during designated "Endocrine Clinic" times.
• Review and triage incoming test results, comparing data with norms to determine patient care needs, notify providers, assist with interventions, and document accordingly.
• Administer medications requiring patient assessments.
• Arrange patient transfers and assess the safety of moving patients.
• Provide patient education on disease processes, prevention, insulin/insulin pumps, and CGM care, including home visits as needed.
• Attend online courses to improve diabetes understanding, with a goal of achieving certification as a Diabetes Care and Education Specialist within 5 years of employment.
• Become cross-trained in the clinic to support workflow and accept assignments in other areas within individualized competency.
• Complete the Annual IHS Diabetes Audit and attend related online training.
• Attend additional training as identified.
• Perform community job site visits, home visits, and hospital visits.
• Perform other duties as assigned.

Requirements

• Current nursing tristate licensure is required.
• Current Basic Life Support (BLS) certification.
• Licensed Practical Nurse (LPN) required; Registered Nurse (RN) preferred.
• Valid Driver’s License and insurable status.
• Must pass background check, physical, and pre-employment drug screen.

Qualifications

• Previous experience in a patient care setting preferred.
• Previous Endocrinology experience preferred but not required.
• Knowledge of diabetic education services.
• Strong organizational and interpersonal skills.
• Ability to determine appropriate course of action in complex situations.
• Ability to work independently, exercise creativity, and maintain attention to detail.
• Ability to manage multiple responsibilities and prioritize work.
• Ability to maintain confidentiality of medical, financial, and legal information.
• Ability to complete work accurately and in a timely manner.
• Excellent verbal and written communication skills.
• Ability to handle difficult situations professionally.
• Ability to present information effectively and respond to questions from providers, patients, and their families.
