About the role
The Outpatient Counselor plays a crucial role in Odyssey House's mission to help individuals and families overcome substance use and mental health disorders. Located at 822 - 953 Southern Blvd., Bronx, NY 10459, Odyssey House offers a range of services designed to support personal rehabilitation, renewal, and family restoration.
Responsibilities
- Administer comprehensive psychosocial assessment tools at intake and throughout the treatment process.
- Provide individual counseling, group counseling, and case management services both onsite and in the client's home or community.
- Collaborate with clients to develop and monitor individualized treatment plans, updating these plans as necessary.
- Maintain up-to-date client clinical records, including treatment planning, group counseling notes, progress notes, and monitoring of social determinants of health.
- Deliver crisis intervention services and conduct ongoing client discharge planning.
- Coordinate with collateral service providers, family members, and criminal/family justice agencies for effective care coordination.
- Deliver clinical services using evidence-based curricula and training materials.
- Act as a liaison with referral agencies and provide regular progress reports.
- Establish community and agency affiliations to enhance service delivery.
- Provide education and direct services to family members regarding their loved one's substance use/misuse and related behavioral issues.
- Collect samples for toxicology testing as needed.
- Attend regularly scheduled staff meetings and case conferences.
- Participate in in-service training seminars and quality improvement activities.
Requirements
- CASAC or CASAC-T certification, or the ability to obtain within 6 months of hire.
- Ability to deliver culturally competent services to a diverse client population in an urban setting.
- Proficiency with computer operations (Microsoft Word, Excel, Electronic Health Records, and Outlook).
- Able to work a flexible schedule.
- Bilingual Spanish proficiency preferred.
