About the role
We are seeking a Clinical Coordinator to assist with all clinical needs for our [LOCATION] clinic patients. The Clinical Coordinator plays a crucial role ensuring the seamless coordination of patient care.
Responsibilities
- Proficient chart preparation
- Communication with patients and providers
- Scheduling of patient visits
- Absessing vital signs and providing hands-on care
- Appropriate utilization and logging of chronic care management activities
- Provider and patient support
Requirements
- Active CMA, LPN or RN license
- At least 3 years of relevant experience
- Excellent communication and interpersonal skills
- Strong problem-solving abilities
- Ability to work effectively under pressure
