About the role
New Jersey Mentor, a part of the Sevita family, provides Residential and Treatment Foster Care services to youth facing various out-of-home placements or transitions. Our mission is to help youth gain stability, comfort, and confidence to overcome challenges and thrive.
Responsibilities
- Facilitate and provide case management and counseling services to an assigned individual caseload.
- Assess referred individuals and develop, coordinate, and evaluate service plans in collaboration with the treatment team, funding sources, and key stakeholders.
- Maintain accurate and comprehensive records in compliance with corporate, contractual, and regulatory standards.
- Monitor and consult with mentors regarding the implementation of service plans, including pre-service introductions and ongoing education.
- Make mentor and family home visits, conduct mentor group meetings, and assist with emergency coverage and crisis intervention support.
- Participate in legal, licensing, regulatory, and certification activities as necessary.
- Engage in activities to enhance and build a network of mentors.
Requirements
- Master’s degree in social work, counseling, psychology, or a related field.
- Two years of related experience working with individuals with developmental disabilities or mental health issues.
- Self-motivated and detail-oriented with the ability to manage multiple tasks effectively.
- Demonstrated skills in motivating teams and achieving goals.
- Valid driver’s license in good standing with access to a registered vehicle and proof of insurance (if providing transportation).
Qualifications
- Experience in working with individuals with developmental disabilities or mental health issues.
