About the role
As a Clinical Coordinator for Reflections Management and Care, you will engage with patients and their families, providing care coordination, advocacy, and educational support. You will conduct comprehensive assessments upon patient enrollment, develop personalized care plans addressing medical, behavioral, and social needs, and collaborate with an interdisciplinary care team to integrate various patient needs and services.
Responsibilities
- Engage with patients and their families, providing care coordination, advocacy, and educational support.
- Conduct comprehensive assessments upon patient enrollment, developing personalized, comprehensive care plans that address medical, behavioral, and social needs.
- Identify social determinants of health during assessments and integrate this knowledge into patient care plans.
- Work collaboratively with an interdisciplinary care team to coordinate and integrate a broad spectrum of patient needs and services.
- Oversee patient case education, ensuring documentation of visits and progress to maintain quality care and compliance with regulations.
- Recruit, train, and oversee staff to maintain high standards of care while fostering a supportive and efficient team environment.
- Design and manage departmental budgets, ensuring resources are used effectively to support patient care and company operations.
Qualifications
- Nursing degree or degree in a health-related field.
- At least 2 years of experience in a clinical coordinator role within a healthcare setting.
- Strong problem-solving abilities and a capacity to implement innovative solutions.
- Excellent organizational skills with meticulous attention to detail.
- Able to work effectively under pressure and in emergency situations.
- Competence in working independently and collaboratively within a team setting.
- Outstanding documentation, communication, and IT skills.
- A passion for maintaining clinical excellence and improving patient outcomes.
- Travel is required: A reliable vehicle is necessary as this position involves travel across multiple counties to serve our clients.
