Clinical Coordinator

About the role

The Clinical Coordinator leads outpatient dialysis care for patients with end-stage renal disease. Key responsibilities include coordinating patient care plans, supervising clinical staff, ensuring safe and compassionate care, and building long-term relationships with patients and families.

Responsibilities

• Coordinate patient care plans and monitor outcomes
• Serve as a supervisor for PCTs
• Ensure safe and compassionate delivery of dialysis services
• Build long-term relationships with patients and families
• Work in a fast-paced, team-oriented environment

Requirements

• Current RN license and CPR certification
• 18+ months RN experience, including 6+ months in dialysis
• Charge RN readiness approval required
• ADN required; BSN preferred
• ICU, ER, or Med/Surg experience preferred
• CNN/CDN certification a plus
• Basic computer skills (MS Word, Outlook)
• Flexible schedule, including weekends and holidays

Qualifications

Preference given to eligible and qualified applicants pursuant to the Navajo Preference in Employment Act.

Strong leadership, communication, and organizational skills are essential.
