About the role

This position is onsite 2-3 days a week in Camden. The Clinical Coordinator will handle patient enrollment services for digital remote monitoring programs (COVID-19, Heart Failure, COPD, Diabetes). Responsibilities include digital platform enrollment, alert monitoring, telephonic patient outreach, education, care coordination, and collaboration with providers and multidisciplinary teams.

Responsibilities

• Facilitate patient identification, screening, and enrollment in the RPM program.
• Collaborate with referring providers on patient enrollment workflow.
• Communicate status updates on RPM program enrollment to providers, inpatient clinical directors, and multidisciplinary teams.
• Review patient agreements, ensure they are signed, and scan them into the patient’s EPIC electronic health record.
• Order digital devices for home delivery from platform partners and confirm receipt of the device kit.
• Coordinate with the program manager to secure phone service for identified patients.
• Facilitate activation of the patient’s MyChart account if not active.
• Confirm device pairing, provide patient education on utilization, and offer continued education as needed.
• Enroll patients in the platform partner’s electronic health record and update as needed.
• Monitor digitally transmitted vital sign results daily (Monday–Friday, excluding holidays).
• Support patients with MyChart account activation if not active.
• Assist patients with remote patient monitoring adherence and program participation.
• Outreach to patients when vital signs are out of range or not transmitted via telephone or video conference, following the escalation pathway.
• Document vital signs weekly and when alerts are out of range in the patient’s CUHC electronic health record.
• Communicate regularly with patients and/or caregivers as needed.
• Document all events and communications in the patient’s CUHC electronic health record.
• Collaborate with the RPM Population Care Coordinator and clinical care team as needed.
• Reinforce the provider’s plan of care and patient-centered care plan.
• Promote chronic disease self-management.
• Coordinate with the RPM program manager and digital platform support services for technical support.
• Facilitate scanning of monthly vital sign summaries into the patient’s EPIC electronic health record.

Requirements

• 2 years of experience.
• LPN License (NJ).
• BLS certification (American Heart Association).
