Clinical Case Manager (CMA/RMA or LPN) - Transitions of Care

About the Role

Full Circle Health is seeking a Clinical Case Manager (LPN or CMA/RMA) to support our Transitions of Care Program across our clinics. The Clinical Case Manager will collaborate with healthcare providers, interdisciplinary teams, patients, and community health partners to coordinate care for individuals with chronic or complex conditions or those requiring high levels of care coordination. This role focuses on assisting with Transitions of Care post-hospitalization.

Schedule

8:00 AM – 5:00 PM, Monday - Friday (daytime shifts).

Qualifications

Education

• LPN from an accredited College of Nursing or completion of an accredited Medical Assistant training program.

Experience

• Minimum of two years of clinical practice.
• Previous experience in population health, case management, or transitions of care preferred.

Licensure / Certification

• Currently certified by American Medical Technologists, American Association of Medical Assistants, American Registry of Medical Assistants, Licensed Practical Nurse, or other accredited certifying organization.
