Clinical Case Coordinator

About the Role

CASES is seeking a highly organized and detail-oriented Clinical Case Coordinator for our Pretrial Services Case Management Team. In this role, you will conduct Supervision Introduction/Orientation meetings with new participants rapidly after their release to review supervision conditions required under their supervision plan. You will assist participants in complying with supervision appointments and court appearances during the pendency of their legal case. The goal is to increase engagement in community treatment services and support participants in achieving their self-identified goals for treatment, recovery, rehabilitation, and crime-free community integration.

This position is based in our Harlem Community Office, with fieldwork in the larger NYC community as needed to assist participants. CASES’ Supervised Release Program (SRP) annually diverts 7,500 people otherwise facing pretrial detention at Rikers Island to community-based supervision and support services. Program participants include youth and young adults, people with substance use disorders and mental illnesses, and individuals who are homeless. SRP participants have not been found guilty of a crime and are not mandated to engage in treatment or other services, requiring staff to tactfully deliver approaches that help participants tap into their intrinsic motivation.

Responsibilities

  • Provide supervision and community-based services to a caseload of pretrial participants.
  • Conduct Supervision Introduction/Orientation meetings with new participants within 24-48 hours of release to review supervision conditions, reporting schedules (based on recidivism risk), and mandatory communication methods (face-to-face, telephone, and texts).
  • Work collaboratively with the court and other pretrial staff to ensure participants’ successful completion of mandated supervision appointments and court appearances.
  • Review legal and court screening documentation and analyze participants’ offending behavior patterns to identify behavioral changes and supports that may reduce the likelihood of pretrial arrests.
  • Conduct ongoing service need assessments to examine psychosocial needs, including the severity of mental and substance use disorders, and barriers to participation and engagement.
  • Link participants to treatment based on eligibility, preferences, and needs, and provide escorts to community services.
  • Ensure the timely flow of community outreach and case management activities to meet program goals.
  • Utilize problem-solving techniques and motivational interviewing to help engage individuals in the process of change.
  • Provide services in a gender-responsive, trauma-informed manner, adhering to the core principles of risk, need, and responsivity.
  • Maintain collaborative relationships with community providers, including supported housing, Health Homes, care management programs, addiction treatment and mental health providers, peer-run providers, education, vocational training, employment services, legal services, medical care, and family providers.
  • Follow program community-based monitoring protocols for high-risk and high-need participants and participate in case reviews for complex cases.
  • Enter assessment, supervision appointment reporting, and progress notes into the program database in accordance with confidentiality regulations and in a timely fashion.
  • Meet monthly case management contact and outreach goals as established by supervisory staff.
  • Provide culturally competent services in accordance with CASES policies and practice.
  • Perform any other duties as required by the Program Director and supervisory staff.
  • Demonstrate flexibility with schedule based on departmental needs.

Requirements

  • Master’s degree in Social Work, Mental Health Counseling, or a comparable professional degree.
  • Licensure preferred.
  • Minimum of three years’ experience working in human services with individuals with substance use disorders, co-occurring mental health and substance use disorders, and involvement in the criminal justice system.
  • Extensive knowledge of community treatment, support services, and resources.
  • Knowledge of the criminal justice system, court processes, and alternatives to detention/incarceration services.
  • Experience using databases such as Salesforce.

Pay

$71,080 per year.

Schedule

Monday through Friday, 9:00 am to 5:00 pm (35 hours per week excluding breaks). Hybrid work arrangement: staff may work remotely one day per week after successfully demonstrating an understanding of their job responsibilities following the first 30 days of onboarding.
