About the role
The Clinical Care Transition Specialist / RN is responsible for selling Option Care Health services to discharging patients, educating them and their families, and ensuring proper placement in the Home Health Care setting.
Responsibilities
- Proactively maintain and grow relationships with referral sources to increase sales and patient starts.
- Serve as key point of contact and representative of Option Care to provide education, assistance, and service to referral sources.
- Conduct assessments of patients selected by the hospital to ensure they are viable candidates for alternative site care.
- Use sales techniques to educate patients, their families, and facility staff about Option Care services and products.
- Provide hands-on, in-person education to patients and caregivers to facilitate timely discharge and therapy independence.
- Communicate with agency staff, medical teams, patients, and family throughout the discharge process to implement an effective treatment plan.
- Collect, review, and complete pre-admission information and secure related signoffs.
- Partner with Account Manager and Regional Sales Director to create and execute area business and growth plans.
- Communicate with Account Manager frequently to discuss opportunities, assess progress, and provide feedback related to promoting Option Care services.
- Deliver educational and promotional programs to patient/caregiver and referral sources.
- Review patient’s medical records to obtain pertinent medical history and insurance information and communicate this to the patient registration department.
- Initiate care transition coordination with referral sources and internal partners to ensure seamless patient transitions to home or alternate site care.
- Maintain confidentiality of patient and proprietary information and observe legal guidelines for safeguarding confidentiality.
- Oversee and provide input to providers regarding patient assessment and treatment process and transition to home care.
- Coordinate with other providers to make necessary arrangements for special medical supplies or appliances.
Qualifications
- Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice with at least 2 years of experience in the healthcare industry.
- Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors.
- Experience applying knowledge of standard practices for all services offered as well as current relevant and applicable standards (i.e. ACHC, URAC standards).
- Experience providing customer service to internal and external customers, including meeting quality standards of services and evaluating customer satisfaction.
- Basic PC skills: Competent use of internet, email, Microsoft Word, Microsoft Excel, Microsoft PowerPoint.
- Experience in identifying operational issues and recommending and implementing strategies to resolve and improve processes.
- Able to plan, organize, and make presentations.
