Clinical Branch Manager - Home Health

Responsibilities

• Supervises, monitors, and ensures competencies and performance of assigned home health care team(s)
• Maintains an accurate evaluation and treatment plan for all patients
• Ensures compliance with medical necessity, homebound status, visit utilization, and documentation of individualized patient assessment and transition planning
• Reviews and provides feedback on electronically submitted documentation to address deficiencies and OASIS corrections
• Participates in and facilitates improvement activities for agency quality and outcome performance
• Serves as an educator and mentor for assigned home health care team regarding home health documentation

Requirements

• 2+ years experience as Home Health Registered Nurse (RN) Supervisor
• Bilingual (Fluent in Spanish) encouraged to apply
• Supervisory experience in Home Health
• Experience with HCHB EMR (preferred)
• Knowledge of Home Health regulations and general healthcare compliance
• Proficiency in all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
