Clinical Assessment Nurse

Role: Clinical Assessment RN Responsibilities: Perform comprehensive in-home assessments using UAS tools for diverse patients.Evaluate medical, functional, and psychosocial needs to develop person-centered care plans.Collaborate with interdisciplinary teams to optimize patient services.Support safe transitions and ensure regulatory compliance.Provide education to patients and caregivers to promote understanding and engagement. Qualifications and Skills: Registered Nurse (RN) licensure.Experience in clinical assessment and in-home care settings.Strong interpersonal and communication skills.Ability to collaborate effectively with diverse healthcare teams.Knowledge of regulatory requirements and care planning processes.
