Responsibilities

Leads integration of Connections to Care (C2C) in the Jobs-Plus program
Coordination of staff trainings and continuation of coaching and supervision, delivering coaching directly as needed
Lead technical assistance engagement of the site(s)
Coordinate pathways to clinical care for program members, and other related functions
Meet and engage members communicating compassion, support, respect and enthusiasm for them and your role as a helper.
Consult with clinician, and others as needed and incorporate recommendations.
Meet all goals set by funder and BronxWorks for position of Clinical and Wellness Coordinator
Perform additional duties and task assigned

Requirements

  • Bachelor's degree in social work (BSW), Psychology, Human Services, Sociology, Public Health, or a related field.
  • Ideal for a recent graduate or early-career professional seeking to build a career in social services, behavioral health, wellness programming, case management, or clinical support services.
  • Five years or more experience (including internships, field placements, volunteer work, or employment) in social services, community programs, mental health, substance use services, workforce development, or related settings preferred.
  • Experience supporting clients through assessments, referrals, care coordination, case management, wellness initiatives, or community outreach is a plus.
  • Strong interpersonal and engagement skills with the ability to build rapport and communicate compassionately with diverse populations.
  • Ability to participate in case conferences, collaborate with multidisciplinary teams, and follow clinical recommendations under supervision.
  • Preferred experience coordinating workshops, trainings, facilitating groups, or supporting staff and program activities.
  • Strong organizational, documentation, and follow-up skills with the ability to manage multiple priorities.
  • Proficiency in Microsoft Office Suite and database systems is required.
  • Ability to work independently, demonstrate initiative, and contribute to a team-oriented environment.
  • Bilingual English/Spanish proficiency is a plus.
  • Ability to read, write, and communicate effectively in English.

Physical Requirements

  • Ability to use a computer for prolonged periods.
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand, walk, or sit for long periods of time.
  • Ability to bend and retrieve objects and/or documents.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
