Clinic Nurse RN with Geriatric Emphasis

About the role

The Clinic Nurse RN with Geriatric Emphasis provides high-quality, patient-centered nursing care within an integrated ambulatory clinic. This role supports a geriatric-focused patient population while functioning as a key member of an interdisciplinary team.

Responsibilities

• Delivers skilled, comprehensive nursing care in alignment with Blacktail Health’s mission, vision, and values, with a particular emphasis on coordinating services for older adults across multiple care settings.
• Supports transitions of care, communicates with Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), and other long-term-care environments, and ensures accurate, timely follow-up on provider orders, care plans, and medication changes.
• Uses clinical judgment, critical thinking, and patient engagement skills to support chronic disease management, Medicare wellness workflows, preventive care, and patient self-management goals.
• Performs nursing procedures, facilitates team-based daily huddles, reviews and reconciles medications, coordinates referrals, and maintains accurate electronic health record (EHR) documentation.
• Participates in geriatric care initiatives such as functional assessments, fall-risk screening, cognitive screening, and advance care planning support.

Requirements

• FMLA eligible
• Must have a Montana RN License in good standing and successfully complete a criminal background check.

Qualifications

• Bachelor's degree in Nursing (BSN)
• Current Montana RN license
• Experience in geriatric care preferred
• Strong communication and collaboration skills
• Comfort working independently and interdependently

DOE $29-$34 based on experience and qualifications.
