Responsibilities
- Develops and implements a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care.
- Maintains aligned, comprehensive plan of care for all participants living in residential facilities and coordinates care between facility and Care/Interdisciplinary Team.
- Conducts a comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Care/Interdisciplinary Team to develop a comprehensive plan of care for each participant.
- Conducts nursing re-assessment semi-annually and as needed.
- Performs general and skilled nursing care to participants as assigned. Implements the plan of care and physician orders in coordination with the Care/Interdisciplinary Team.
- Observes, records, and reports participant’s condition and reaction to drugs and treatments to providers.
- Triages participants to determine priority of care and assists in the treatment, examination, and testing of participants.
- Participates in nursing on-call rotation to provide after-hours and weekend assistance and care, as needed, to PACE participants.
Requirements
- Graduate of a school of professional nursing.
- Current Colorado Registered Nurses (R.N.) License, Cardiopulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
Qualifications
- Must be a U.S. citizen or permanent resident.
- Must be able to pass a background check.

- Knowledge of PACE program requirements and regulations.

Pay
Pay Range: $37.45 - $44.95 per hour based on experience.
Schedule
Friday 8am - 5pm and participation in on-call rotation (evenings and weekends); occasional weekends.
