About the role

Logansport State Hospital is a state-operated inpatient psychiatric hospital for adults with mental illness. Using an interdisciplinary approach and evidence-based programming, we promote recovery and return of the individual to the community.

Responsibilities

  • Prioritize and organize the daily care of individuals to ensure consistent and effective service delivery.
  • Make care assignments and assist in creating treatment plans in collaboration with other clinical staff to meet individual needs.
  • Provide direct observation of individuals with specific monitoring situations (e.g., line-of-vision or one-on-one precautions) to ensure safety and therapeutic engagement.
  • Direct support staff to coordinate workflow and communicate problems to unit supervisors to maintain smooth operations.
  • Complete required documentation of nursing notes, transfer forms, and other records to ensure accurate and timely reporting.
  • Provide orientation and support to staff on the unit to maintain continuity and quality of care.
  • Provide ongoing assessment and documentation of individuals' physical and psychosocial status from admission to discharge to inform care decisions.
  • Administer medications and treatments according to state-operated health care facility policies and procedures to support recovery and wellness.
  • Develop and implement nursing care plans for acute medical problems to address evolving needs.
  • Complete medication administration supervision monitors and clinical observation notes for support staff to ensure compliance and quality.
  • Educate individuals and family members regarding medication, procedures, and treatment goals and objectives to promote understanding and engagement.
  • Respond and intervene in emergency situations and with agitated individuals to maintain safety and therapeutic environment.
  • Ensure proper safety and infection control standards are met for the unit to protect staff and individuals.

Requirements

  • Deliver high-quality work with effective verbal and written communication skills, ability to proofread and edit documentation for clarity and accuracy, and display technical expertise in understanding and applying medical and facility terminology.
  • Ability to use MS Office applications and electronic patient records.
  • Proficiency in computer skills and ability to learn programs used in maintaining electronic medical records.
  • Drive results with proficiency in computer skills and ability to learn programs used in maintaining electronic medical records.
  • Show care and understanding with additional knowledge and/or ability may be required based on the population the facility serves (e.g., children, adolescents, geriatrics).
  • Experience & Education: Must possess an Associate or Bachelor’s degree in nursing from an accredited college. Must have a license to practice nursing in the State of Indiana as a Licensed Practical Nurse. Must hold and maintain CPR certification. Must stay current with behavioral intervention training.
