Responsibilities

  • Perform assigned nursing procedures and provide general nursing care to participants, in accordance with InnovAge policies and all applicable regulations.
  • Provide general nursing care which includes the supervision or administration of prescribed medications and treatments in accordance with nursing standards.
  • Furnish services, treatments and diagnostic and preventive procedures as assigned per the plan of care, in accordance with agency policies and applicable regulations.
  • Perform treatment(s) per policies and procedures.
  • Implement the orders written by primary care and nursing orders when necessary.
  • Perform phlebotomy functions when necessary.
  • Sign off on orders, and explain orders and instructions to appropriate personnel, implement orders written by primary care and nursing when necessary.
  • Administer vaccinations and TB testing, document and record results test administration and results appropriately.
  • Prepare clinical and progress notes and complete assigned audits and perform electronic or written documentation.
  • Adhere to Agency standards and consistently interpret and accurately perform all assigned responsibilities.
  • Perform Medication Room Nurse function as assigned.
  • Deliver routine nursing care to PACE participants and related services within the scope of practical nursing license. This includes, but is not limited to: wound care, treatments, weights, vital signs, and assisting with nutritional needs.
  • Provide assistance in medical emergencies under the supervision of the Primary Care staff or Registered Nurse.
  • Assist with the orientation of new LPNs and clinic support staff as needed.
  • Complete appropriate and timely documentation of all nursing care provided as well as medication administration.
  • Additional work functions and duties may be assigned.

Requirements

  • Current State Issued Licensed Practical Nurse (LPN) license.
  • 1+ yr as an LPN in a clinic or long term care setting with geriatrics experience.
  • Current First Aid and BLS certifications are required prior to hire. Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross.
  • High School Diploma or GED.

Preferred

  • Knowledge of medications used in the treatment of geriatrics.
  • Bi-lingual.

Pay

Posted Pay Range: $31.01 - $38.08. Pay may vary depending on job related factors, such as work location, experience, knowledge, skills, education, certifications, training and internal equity.
