Responsibilities
- Provide physical, occupational, or speech therapy services while leading your team in delivering high-quality care
- Mentor and support clinicians, helping them grow in their clinical skills and confidence
- Partner with community leadership to implement and strengthen the EmpowerMe 360 model
- Coordinate care across therapy, wellness, and medical services to ensure a seamless resident experience
- Lead and support team performance, scheduling, and day-to-day clinic operations
- Contribute to program development and quality improvement
Qualifications
- Licensed therapist in physical, occupational, or speech therapy
- Experience in care coordination or related field preferred
- Strong leadership and interpersonal skills
- Ability to work independently and as part of a team
- Passion for improving the lives of seniors and their communities
