Clincical Social Work-LCSW-Care Management (Heme/Onc)

Position Summary

Utilizing clinical social work techniques and theory, consults on, assesses, and intervenes with high-risk patients and their families regarding the impact/amelioration of emotional and social problems.

Duties & Responsibilities

• Affirms patient and family to ascertain emotional and social problems.
• Utilizes the department’s psychosocial assessment tool as a guideline.
• Identifies issues needing immediate attention and formulates a plan.
• Reports suspected cases of abuse and neglect.
• Contacts family members during the initial assessment process.
• Collaborates with healthcare team members to address patient’s emotional and social needs.
• Provides psychotherapeutic treatment and psychosocial support to patient and family.
• Maintains regular contact with inpatients and families.
• Uses social work techniques and theory to assist patient and family in coping with illness or social problems.
• Begins assessment for inpatients within 24 hours of referral.
• Maintains confidentiality and practices professional discretion.
• Documents each patient encounter in the medical record or department clinical record.
• Serves as clinical supervisor for other department personnel.
• Complies with departmental, regulatory, and professional requirements for documentation and data collection.
• Participates in department programs and complies with policies and procedures.
• Supports graduate and undergraduate training programs by supervising MSW and BSW students.
• Identifies and recommends improvements to department operations.
• Represents department throughout hospital and outside community.
• Maintains work area and equipment in a neat and orderly manner.
• Notifies supervisor of patients not regularly served.
• Performs other duties as needed.

Requirements

• Master’s Degree from an accredited school of Social Work.
• Massachusetts Licensed Certified Social Worker (or obtained within six months of employment).
• Knowledge of Social Work process and community resources and agencies in relation to health, illness, and disability.
• Advanced interpersonal skills to interact effectively with various healthcare professionals, develop and maintain therapeutic relationships with patients and families, and supervise graduate students.
• Cultural sensitivity and comfort with a wide range of racial and ethnic populations.

Qualifications

• Computer literacy with ability to use standard word processing, spreadsheet programs and e-mail (i.e. MS Word, Excel, Outlook).

• Interpersonal skills necessary to interact effectively with various healthcare professionals, to develop and maintain effective therapeutic relationships with patients and families, and to supervise graduate students.
