Chronic Illness Nutrition Educator - Registered Dietitian

About the role

This position reports directly to the Director of Nursing. The Chronic Illness Nutrition Educator provides interdisciplinary support with the healthcare team to improve health and promote wellness, focusing on Medical Nutrition Therapy for diabetic and other chronically ill adult patients to enhance health and patient outcomes.

Travel to all Kintegra sites and community events is required to support public needs.

Direct report: Diabetes Care and Education Specialist.

Responsibilities

  • Assess, evaluate, and implement individual health/behavioral plans for improving chronic illness, including Diabetes Type II.
  • Document accurately and timely in the Electronic Medical Record.
  • Possess a basic understanding of billing related to the role of RD/CDE.
  • Facilitate group classes to meet American Diabetes Association requirements for Kintegra to maintain recognition as an education site.
  • Provide weight management therapy for adult patients with unhealthy BMI, with or without other associated illness.
  • Provide education to meet communal needs, promoting health and identifying healthy resources for patients.
  • Participate in continuous quality improvement.
  • Perform other duties as assigned.

Qualifications

  • Bachelor of Science (BS) or higher in a relevant field.
  • Certification(s)/Licensure: Registered Dietitian (RD), Certified Diabetes Educator (CDE), and BLS.

Experience

  • 3 years of Outpatient Dietary Management with a focus on improving chronic illness/disease prevention.

Skills

  • Able to sit, stand, and walk for long periods of time.
  • Able to read and understand the English language.
  • Able to effectively maintain confidentiality of health information and communicate with all levels of personnel.
  • Knowledge and understanding of chronic illness and health promotion.
