Position Summary
The Chronic Care Nurse manages high-risk patients by executing personalized care plans, providing health education, and coordinating with interdisciplinary teams. This role focuses on preventing hospitalizations, promoting self-management, and significantly improving the overall quality of life for chronically ill patients.
Key Responsibilities
- Assist and develop care plans for patients who have chronic conditions.
- Evaluate and educate patients on optimizing their health with chronic conditions.
- Communicate and coordinate care with an interdisciplinary care team.
- Contribute to a team to meet value-based care and improve the overall population health.
- Telephone triage skills – patient call backs and pharmacy call backs.
Qualifications
- Education/Experience: - Graduate of an accredited school of nursing.
- 6 months to one-year related experience in a rural medical clinic or private medical practice preferred.
- Critical thinking and problem-solving skills are essential to providing excellent customer service and communication with patients.
- Ability to interpret clinical documentation across varying levels of care.
- Computer Skills are required.
- Strong organizational and time management abilities.
- Previous Chronic Care Management experience preferred.
