Chronic Care Nurse – RN/LPN

Position Summary

The Chronic Care Nurse manages high-risk patients by executing personalized care plans, providing health education, and coordinating with interdisciplinary teams. This role focuses on preventing hospitalizations, promoting self-management, and significantly improving the overall quality of life for chronically ill patients.

Key Responsibilities

• Assist and develop care plans for patients who have chronic conditions.
• Evaluate and educate patients on optimizing their health with chronic conditions.
• Communicate and coordinate care with an interdisciplinary care team.
• Contribute to a team to meet value-based care and improve the overall population health.
• Telephone triage skills – patient call backs and pharmacy call backs.

Qualifications

• Education/Experience: - Graduate of an accredited school of nursing.
• 6 months to one-year related experience in a rural medical clinic or private medical practice preferred.
• Critical thinking and problem-solving skills are essential to providing excellent customer service and communication with patients.
• Ability to interpret clinical documentation across varying levels of care.
• Computer Skills are required.
• Strong organizational and time management abilities.
• Previous Chronic Care Management experience preferred.
