Compensation Structure

  • Productivity-based pay structure:
    • $10.00 per completed patient encounter up to 99 encounters/month
    • $10.25 for 100–149 encounters/month
    • $12.00 for 150–199 encounters/month
    • $14.00 for 200–249 encounters/month
    • $16.00 for 250+ encounters/month
  • Payment tier increases require 2 months consistency to achieve and are awarded in the third month
  • $1/encounter incentive compensation for bilingual contractors (equivalent to $3/hr), applied only if hired into a bilingual position
  • Monthly outreach time (chart review, contact attempts, actual call time, care coordination, documentation/billing) is billed in 20-minute units (“encounters”), up to 3 encounters per patient per month:
    • 20–39 min = 1 encounter
    • 40–59 min = 2 encounters
    • 60+ min = 3 encounters
  • No compensation outside billable encounters except goal bonuses, referral bonuses, and employee engagement monetary prizes
  • No pay for onboarding until billable encounters begin; onboarding is self-led (2–3 hrs initial orientation + 1–2 days workflow training) and may take 3–14 days depending on schedule

What Your Impact Will Be

  • Abide by the plan of care and orders of the practice
  • Provide prevention and intervention for multiple disease conditions through motivational coaching
  • Develop positive interactions with patients on behalf of practices
  • Improve revenue by creating billable Care Management episodes and increasing visits for chronic condition management
  • Develop detailed care plans for both doctors and patients for prevention and intervention
  • Understand health care goals associated with chronic disease management provided by the practice
  • Attend regularly scheduled meetings (Monthly Clinical Update Meeting, monthly 1:1 with supervisor, etc.) to define current scope of work

Requirements

  • Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.)
  • Current COMPACT license to practice as RN/LVN/LPN in current state of residence with no disciplinary actions, OR licensed in the non-compact state where the applicable practice is located
  • Minimum of two (2) years of clinical experience in a clinic setting, Med/Surg, Case Management, and/or home health care
  • Hands-on experience with Electronic Medical Records
  • Understanding of Windows desktop and applications (Microsoft Office 365, Teams, Excel, etc.)
  • HIPAA-compliant area in home to conduct Care Management duties
  • Ability to exercise initiative, judgment, organization, time-management, problem-solving, and decision-making skills
  • Ability to quickly learn new technology through video tutorials and resources in a self-led environment
  • High-speed internet (minimum download 18 Mbps / upload 6 Mbps)
  • Desktop or laptop computer in good working order with Windows or Mac operating system (no Chromebooks, iPads, or tablets)
  • Excellent verbal, written, and listening skills

Patient panel expectation: Minimum of 30–50 patients per calendar month within first three months (depending on patient availability). Care Managers must complete due diligence on 100% of assigned patients and billable encounters on 90% unless patients are unable to participate due to health conditions.
