About the role
Provide comprehensive care to patients with chronic conditions, coordinating medical services and managing medications.
Responsibilities
- Assess patient needs and develop care plans
- Monitor patient health status and adjust care plans accordingly
- Collaborate with healthcare providers to ensure continuity of care
- Manage patient medication schedules and educate patients on proper usage
- Document patient interactions and progress in electronic health records
Requirements
- Bachelor’s degree in Nursing or related field
- Current licensure as a Registered Nurse in applicable state(s)
- Experience in chronic care management
- Strong interpersonal and communication skills
- Proficiency in electronic health record systems
Qualifications
- Excellent organizational and time management skills
- Ability to work independently and as part of a team
- Knowledge of common chronic conditions and treatment options

- Nursing knowledge and clinical skills
