CHOICES Care Coordinator - Tennessee Residents only
About the Role
A field-based, comprehensive case-management position for Tennessee residents who will travel weekly within an assigned coverage area. As a CHOICES Care Coordinator, you will partner with members and their families to identify supports and services that help them achieve personal goals for health, employment, independence, and community living. You will assess needs, develop person-centered care plans, coordinate services, monitor outcomes, and advocate for access to resources that promote long-term success and quality of life.
This role requires conducting in-person visits, assessing members' physical, behavioral, functional, psychosocial, financial, and long-term care needs, and collaborating with a multidisciplinary team to coordinate services that ensure safety, independence, and well-being.
Responsibilities
- Partner with members and families to identify needed supports and direct services to meet personal goals for health, employment, and independent or community living.
- Collaborate with a team of clinical and social support colleagues to address the physical, behavioral health, and long-term service needs of each member.
- Conduct thorough, objective face-to-face visits to assess each member’s situation, including physical, behavioral, functional, psychosocial, financial, and employment/independent living expectations.
- Utilize criteria to authorize appropriate home and community-based services and confirm those services are being provided and meeting members’ needs.
- Maintain a valid driver’s license and adhere to travel requirements within the assigned service area.
- Complete a TB skin test (applies to field-based coordinators).
- Undergo various immunizations and/or associated medical tests as required for the position.
- Complete a digital literacy assessment.
Note: Employees in this role are required to remain in the position for 24 months before becoming eligible to apply for other internal positions.
Requirements
- At least 2 years of clinical or case management experience.
- Active Registered Nurse (RN) license in the state of Tennessee or a license in a state participating in the Nurse Licensure Compact Act or an active unrestricted Master’s-level social work license (LCSW, LMSW, or LAPSW).
- Strong assessment, care coordination, customer service, and case management skills.
- Critical thinking and problem-solving abilities.
- Excellent communication, interpersonal, and organizational skills.
- Effective time management skills and the ability to work independently.
- Basic Microsoft Office, email, and digital literacy skills.
- Valid driver’s license and willingness to travel throughout the assigned service area.
- Residency within the state of Tennessee.
Schedule
- Monday through Friday, 8:00 AM to 5:00 PM (CST or EST).
- No on-call responsibilities.
- Compressed work week options may be available with management approval after 1 year of employment.
Additional Notes
Employees required to operate a vehicle for company business (whether BCBST-owned, personal, or rental) on a routine basis will be enrolled in the BCBST Driver Safety Program. This includes maintaining a valid driver’s license, auto insurance compliance with minimum liability requirements, and an acceptable motor vehicle record (MVR).
