About the Role
A field-based, comprehensive case-management position for Tennessee residents who will travel weekly within an assigned coverage area. As a CHOICES Care Coordinator, you will partner with members and their families to identify supports and services that help them achieve personal goals for health, employment, independence, and community living.
Responsibilities
- Partner with members and families to identify needed supports and direct services to meet personal goals for good health, employment, and independent or community living.
- Collaborate with a multidisciplinary team of clinical and social-support colleagues to meet each member’s physical, behavioral-health, and long-term-service needs.
- Conduct thorough, objective face-to-face visits to assess each member’s situation, including physical, behavioral, functional, psychosocial, financial, and employment/independent-living expectations.
- Utilize criteria to authorize appropriate home- and community-based services and confirm that services are being provided and member needs are being met.
- Maintain a valid driver’s license and travel routinely to members’ homes, nursing facilities, hospitals, and other community settings.
- Complete a TB skin test (applies to field coordinators).
- Fulfill various immunization and/or associated medical-test requirements for the position.
- Pass a digital-literacy assessment.
Requirements
- At least 2 years of clinical or case-management experience.
- Active Registered Nurse (RN) license in the state of Tennessee or a license in a Nurse Licensure Compact state, or an active unrestricted Master’s-level social-work license (LCSW, LMSW, or LAPSW).
- Residency in the state of Tennessee.
- Valid driver’s license and willingness to travel throughout the assigned service area.
- Basic Microsoft Office, email, and digital literacy.
- Strong assessment, care-coordination, customer-service, and case-management skills.
- Critical-thinking and problem-solving abilities.
- Excellent communication, interpersonal, and organizational skills.
- Effective time-management skills and the ability to work independently.
Schedule
- Monday through Friday, 8:00 AM to 5:00 PM (CST or EST).
- No on-call responsibilities.
- Compressed work-week options may be available with management approval after one year of employment.
