About the role
The Apartment Counselor provides direct care service to consumers in the CHIPP Supported Living Program. The Mental Health Department delivers recovery-oriented services based on consumer strengths and choice.
Schedule
Full-time Apartment Counselors work a rotating shift of three working days followed by six days off.
- Weekdays (Sunday–Thursday): 4:00 p.m.–11:00 p.m. (7 hours work), 11:00 p.m.–7:00 a.m. (8 hours sleep), 7:00 a.m.–9:00 a.m. (2 hours work)
- Weekends (Friday–Saturday) and Holidays: 4:00 p.m.–11:00 p.m. (7 hours work), 11:00 p.m.–7:00 a.m. (8 hours sleep), 7:00 a.m.–4:00 p.m. (9 hours work)
Responsibilities
Critical Elements
- Medication Monitoring: Implement the medication monitoring system (monitor medications taken, complete medication logs, etc.)
- Direct Care: Respond to consumer emergencies during the shift and follow program situation management directives. Provide supportive counseling, educational and recreational activities, and assistance with daily living skills (cooking, cleaning, laundry, socialization)
- Documentation: Accurately complete required documentation (daily progress notes, incident reports, communication logs, PRN reports, etc.)
- Consumer Safety: Conduct periodic safety checks on consumers during the shift
Additional Elements
- Participate in agency staff meetings, program staff meetings, trainings, and special activities
- Contact supervisor for any emergency
- Transport CHIPP consumers as needed
- Serve as an active team member of the CHIPP Supported Living Program (assist co-workers with site coverage, train new apartment counselors, and support program projects)
- Complete a minimum of 24 hours of training per fiscal year
- Participate in quality improvement and consumer education efforts
- Follow and support agency philosophy, policies, and procedures
- Assume other duties as assigned by the Supported Living Program Coordinator or Assistant Supported Living Program Coordinator
Requirements
- One year of college credits in Social Science or one year of human service experience
- 21 years of age or older
- Valid Pennsylvania Driver’s License held for at least two years
- Reliable transportation
- Pennsylvania State Police Background Check
Pay
$13.00–$13.00 per hour
