Children's Social Worker, Part-Time

About the role

Assist patients who have complex psychosocial needs, and requiring assistance with eligibility determination for social programs and funding sources as well as qualification for community assistance from a variety of special funds and agencies. Offer short-term counseling and crisis intervention to patients and families with psychosocial needs and coordinate and facilitate the development of a safe discharge plan of care for high-risk patient populations.

Responsibilities

• Work in conjunction with the RN Case Manager to manage socially complex patients identified by initial assessment and referral
• Identify the optimal resources available to meet the ongoing needs of the patients and their care givers along the care continuum
• Initiate this plan on admission

Education

• Required: Graduate from an accredited Social Work Program with Bachelors in Social Work
• Preferred: MSW

Experience

• Required: Clinical rotation experience can be taken into consideration in lieu of experience if it is applicable to the job being applied for
• Preferred: Experience in hospital social work
• Knowledge of managed care practices, community resources, regulatory and insurance reimbursement issues preferred
