Child Welfare Case Manager

One Hope United works with children of all ages who are wards of the state due to abuse, neglect or abandonment. These children need a stable, dependable, forever home and a family they can call their own. Safe, well-educated and well-cared-for children become adults who contribute to a more prosperous community. Case Managers recruit and train foster parents, provide supportive services to children and families, and ensure continuity of care focused on resolving or preventing child abuse or neglect.

About the role

The Case Manager provides assessment, planning, linkage, coordination, monitoring, evaluation, and advocacy to children, families, foster families, and primary caregivers. They work as a team member with therapists, foster parents, guardians ad litem, and other professionals to develop and maintain direct working relationships with mental health professionals, community agencies, and the child’s support system.

Responsibilities

• Abides by Incident Reporting policy and procedures as applicable
• Arranges visitation schedules, supervises visitations, and documents observations
• Assists in providing appropriate information and accompanies children to appointments as necessary
• Helps primary caregivers and/or foster families implement tasks and activities according to the case plan
• Attends and participates in professional development training, staffing reviews, administrative reviews, judicial reviews, permanency staffing, school staffing, and CPT staffing
• Completes all non-clinical intake paperwork for case management services according to policy
• Completes and attends mandatory trainings
• Develops and maintains appropriate documentation and concurrent planning
• Identifies strengths, limitations, risk factors, and needs to coordinate services for the child and families
• Intervenes and assists staff in crisis situations
• Maintains client confidentiality and obtains necessary releases of information
• Maintains caseload productivity and standards for direct client contact
• Maintains face-to-face and telephone contact with assigned children/families in appropriate settings
• Notifies biological parents of changes in status, placement, medical, and clinical treatment
• Provides written assessments and develops case plans in a timely manner, updating them weekly with the team
• Monitors and reviews case plans, filing changes with the court, DCF, and Lead Agency
• Provides complete documentation for services rendered within established time frames
• Offers ongoing support to primary caregivers and/or foster families
• Provides recommendations for family reunification, termination of parental rights, or case closure
• Refers children for medical care, including developmental, psychiatric, and medical evaluations
• Tracks court dates, attends court with prepared documentation, and assists with notifications
• Performs other related duties as assigned

Requirements

• Bachelor’s degree required; Master’s degree in Social Work, Criminology, Psychology, Sociology, Counseling, Special Education, Education, Human Development, Child Development, Family Development, Marriage & Family Therapy, or Nursing preferred
• State of Florida Child Welfare Certification (CWCM) preferred; must be willing to obtain within 1 year of employment
• Must complete a 10-week training program
• Must have reliable transportation and be willing to drive daily for work-related purposes
• Availability to include evenings, weekends, and on-call required
• Ability to work in a culturally diverse environment required
