Child and Family Support Specialist - School Based

About the Role

The SBS Child and Family Support Specialist (SBS-CFSS) serves as an active member of a multidisciplinary team providing assistance and support to facilitate clients’ behavioral, emotional, and social development and learning in school and within the community. This includes co-facilitating daily social and emotional support interventions to decrease recurrence of behavioral health-related issues and absenteeism. The CFSS also provides case management services to students and their families, both in school and in the community, who need additional wrap-around support. Case management may include treatment planning, advocacy, linkage, referrals, and crisis intervention.

Responsibilities

• Respond to students’ social and emotional needs alongside school staff when appropriate.
• Facilitate debriefing for school staff, students, and caregivers following behavioral incidents, disciplinary outcomes, or crisis interventions.
• Support students, caregivers, and school staff with transitioning back to school after behavioral health-related crises (e.g., ED visits, hospital admissions) or disciplinary outcomes (e.g., suspensions).
• Provide case management services, including communication with schools, pediatricians, DCF, juvenile court, and other social service agencies.
• Support client recovery plan goals.
• Provide crisis intervention to children and families in the home, office, or community as needed.
• Offer advocacy and assistance with linkage to services.
• Collaborate with the integrated CMHA clinical team to ensure effective wrap-around services.
• Maintain timely documentation per CMHA policies, including progress notes, treatment plans, authorizations, consent forms, activity logs, and monthly targeted case management notes.
• Maintain an appropriate caseload as determined by CMHA and funders.
• Perform other related duties as assigned.

Principal Customers

• Students in grades K-12 in contracted public school districts and their families.

Critical Work Relationships

• CMHA programs, schools, pediatricians, DCF, juvenile court, and other social service agencies.
