Charge Registered Nurse, Rehab, Full time Nights, Clarksville

Key Responsibilities

• Performing the initial nursing assessment, communicating findings with the assigned physician or mid-level practitioner.
• Absessing the wound stage and measurements, documenting care plans, evaluating the patient status in regards to desired outcomes.
• Identifying patient care situations that require intervention.
• Participates in quality improvement initiatives, providing education to the patient and/or caregiver, and serving as patient advocate in the delivery and coordination of care.
• Collaborates as needed with other healthcare providers, Wound Care Center Providers, the Program Director and Medical Director regarding patient needs.
