Chaplain (Hospice)
Job Summary
The Chaplain will provide pastoral or spiritual counseling in accordance with patient/family beliefs and Ascend Hospice policies and procedures.
Responsibilities
- Provides pastoral or spiritual counseling in keeping with patient/family beliefs, and in accordance with Ascend Hospice policies and procedures.
- Completes spiritual/pastoral assessment as indicated and appropriate, completed within 1 day of admission.
- Develops an individualized spiritual/pastoral plan of care, according to regulations.
- Offers patients and families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs and values.
- Responds to hospice patient/family needs as requested.
- Collaborates with other patient/family care services to ensure a continuum of care, including continuity of care following death or discharge.
- Prays with families and patients, if and when desired by the patient and/or family unit.
- Participates as a member of the multidisciplinary Hospice team, providing quality Hospice care to the patient and his/her family.
- Initiates bereavement services, in accordance with Ascend Hospice policies and procedures.
- Documents a plan of care for the bereaved survivors, based on the IDT's assessment.
Qualifications
Master's Degree in Divinity, 2-4 years of Chaplain experience with end of life care, at least 1 unit of CPE completed, excellent written and verbal communication skills.
