Founded in 1972, RiseWell Community Services (RiseWell) is an innovative, multi-service, community-based social wellness agency. We are a major provider of health and wellness services, senior and children’s care, addiction recovery services, as well as affordable housing and support. RiseWell serves thousands of community members across Suffolk, Nassau, Queens, Brooklyn, Bronx, and Manhattan. Our programs are designed to meet the needs of vulnerable populations, including individuals in recovery, those living with chronic medical conditions, adult home residents, the homeless, low-income seniors, and at-risk children.
About the Role
Our Children and Family Treatment Support Services strengthen children, youth, and their families to help them make informed decisions about their care through evidence-based practices. Primary goals include rebuilding relationships, enhancing communication, learning coping skills, incorporating therapy into everyday practice, and conducting assessments for mental health and/or substance use needs.
In this role, you will provide services to children and youth under 21 years old, offering community-based support in non-institutionalized settings to enable them to remain at home and in the community. You will offer participants independent choices among an array of service options and providers, including individual and family therapy (OLP services).
Responsibilities
- Provide individual, group, or family therapy, family peer support, and rehabilitation services.
- Deliver community-based support to children and youth in non-institutionalized settings.
- Conduct assessments for mental health and/or substance use needs.
- Support families in rebuilding relationships, enhancing communication, and learning coping skills.
- Offer participants choices among service options and providers.
Requirements
- Licensed Master Social Worker.
- Experience working with individuals in recovery from mental illness.
- Valid and clean NYS Driver’s License.
- Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.
