About the Role

A Certified Registered Nurse Practitioner (CRNP) provides primary care services, including assessing and managing patients in the clinic through the patient-centered medical home concept, following established standards and practices. The position combines the roles of provider, mentor, educator, researcher, and administrator. The CRNP must honor the Commonwealth of Pennsylvania’s Nurse Practitioner’s guidelines and ethical requirements, exercise effective communication skills, and comply with confidentiality, privacy, and security standards for protected health information. The CRNP must also be willing to travel to other clinics to provide care and coverage as needed.

The position reports to the Supervising Physician for medical issues, the Practice Manager and Co-Directors of CRNP & PA Services for practices and policies, and the Medical Director of the clinic. No staff report to this position.

Responsibilities

• Obtain, compile, and record patient medical data, including health history, progress notes, and physical examination results.
• Interpret and integrate data to determine appropriate treatment plans.
• Instruct and counsel patients; communicate clearly with patients and healthcare workers.
• Collaborate with physicians in managing acute and long-term medical needs.
• Order and interpret diagnostic tests (e.g., x-ray, electrocardiogram, laboratory tests).
• Diagnose and treat acute, chronic, and long-term healthcare issues.
• Interview and advise patients on health and illness prevention; recommend resources.
• Provide counseling, instruction, and patient education on treatment and follow-up care.
• Assist in supervision of Medical Assistants and Licensed Practical Nurses.
• Identify, evaluate, and address disease prevention and health promotion.
• Participate in meaningful use data and document in the electronic health record.
• Administer emergency care per BCLS, ACLS, or PALS standards.
• Document all patient encounters in compliance with EMR processes.
• Adhere to “incident-to” and “non-incident-to” service and billing guidelines.
• Exercise HIPAA confidentiality and security measures.
• Participate in continuing education and quality improvement activities.
• Serve as a clinical resource for staff, clients, and families, including triage support.
• Practice as an outpatient provider with 32 hours of patient care per week (3 slots/hour, 96 slots/week) and 8 hours of administrative time, totaling 40 hours on-site weekly.
• Maintain zero charts overdue >10 days and no more than 100 at month-end.
• Monitor lab and document box of other providers as assigned.
• Work late nights (1–2 per week) and participate in outpatient call schedule (1 weekend/month, occasional weekdays).
• Participate in weekend rotation (up to 2 Sundays or Saturdays per month, or a combination).
• Work 1–2 holidays per year (with a compensatory day off within 90 days).
• Cover colleague time off and staffing emergencies as needed.
• Participate in nursing home on-call coverage.
• Work independently and collaboratively with medical staff for consultation, collaboration, or referral.
• Complete timely patient care documentation and required evaluations.
• Complete Peer Reviews as assigned by the Credentialing department.
• Perform other duties as assigned.

Medication Administration

• Administer medications and injections, including Tetanus prophylaxis and non-prescription medicine.
• Prescribe and dispense medications relevant to the area of practice.
• Prescribe and dispense Schedule II medications with a DEA certificate.
• Prescribe and dispense Schedule III, IV, and V medications with a DEA certificate.

Basic Invasive Procedures

• Cautery and epistaxis management.
• Urethral catheter insertion and removal.
• Collection and preparation of specimens (e.g., gram stains, C&S).
• EKG tracings and Pap smears.
• Foreign body removal (ears, nose, throat, rectum, soft tissue).

Ophthalmologic Procedures

• Visual and corneal screening.
• Simple removal of foreign bodies from the conjunctiva.
• Visual acuity assessment.

Surgical Procedures

• Subcutaneous local and digital anesthesia.
• Repair simple lacerations; laceration management (cleansing, irrigation, edge revisions, suturing).
• Suture and staple removal.
• Treatment for minor superficial (1st and 2nd degree) burns.
• Incision and drainage of abscesses; wound irrigation and packing.
• Control of external hemorrhage.
• Carry out aseptic and isolation techniques.

Care Coordination

• Maintain documentation on care coordination to ensure patient safety and follow-through.
• Assist patients in problem-solving issues related to healthcare, financial, and psychological barriers.
• Serve as the system navigator and point-of-contact for high-risk patients and families.

Requirements

• Meet The Wright Center for Community Health and its affiliated entity’s EOS© People Analyzer Tool.
• Buy-in and experience working in the EOS® model (strongly preferred).
• Mission-oriented; professional representation of the organization.
• Licensed by the State of Pennsylvania as both a Registered Nurse (RN) and a Certified Registered Nurse Practitioner (CRNP).
• Valid DEA Registration Certificate and PA CRNP Prescriptive Authority.
• Completion of an accredited, Board-approved master’s or post-master’s nurse practitioner program or equivalent.
• Demonstrated expertise in relevant medical practices, protocols, and trends.
• Ability to manage multiple responsibilities and emergency situations.
• Proficiency in office management, computer software, and office machines.
• Familiarity with electronic health records (preferably Medent).
• Current BLS, ACLS, and PALS certification.
• Valid PA driver’s license.
