Requirements
- Certified Radiology Technologist
- Radiology experience: 1 year
- License: RAD‑AZ (copy needed)
- Certifications: BLS‑AHA (copy needed); CERT‑ARRT‑R (copy needed)
- SSN required
- Date of birth required
- Work history: no more than 30 days of gap in entire work history
- References: two references – one within the past 12 months (most important) and one within the past 3 years
