About the role
Serve as a bridge between rural residents of Cumberland County and clinical health care service providers for hepatitis C (HCV) screening, treatment, and educational support.
Responsibilities
- Work with the Project Director/Co-director to support mobile health unit operations and outreach, including scheduling and deployment of a mobile liver health unit in rural communities of Cumberland County.
- Conduct outreach to engage residents of Cumberland County in hepatitis C screening, treatment, and education.
- Participate in specialized events.
- Provide intake assessments, HCV (and HIV) rapid testing, confirmatory testing, referral to care, care coordination, SDOH assessment and wraparound services, and implementation of the Hepatitis C Self-Management Program.
- Facilitate care coordination and patient navigation to SUD treatment, health care clinics, preventive screenings, and mental health services. Schedule appointments, arrange transportation, and assist with referrals to insurance enrollment.
- Maintain accurate records and documentation of outreach contacts, appointments, and services offered using electronic records.
- Participate in data collection for evaluation of the Liver Health Mobile Unity project.
- Assist in maintaining supplies on the mobile health unit and developing a welcoming environment on the unit for Cumberland County residents.
- Provide ongoing patient support using a Hepatitis C Self-Management Curriculum developed for the project to maximize cure rates.
- Work with members of the Hepatitis C Elimination Service Corps who will assist in screening residents for chronic diseases.
- Participate in all training required for the Rural Health Transformation grant program.
- Attend meetings as required.
- Understand and adhere to Rowan-Virtua SOM compliance standards as they appear in the Corporate Compliance Policy, Code of Conduct, and Conflict of Interest Policy.
- Keep abreast of all federal, state, and Rowan University regulations, laws, and policies as they presently exist and as they change or are modified.
Requirements
- This successful candidate will work on a mobile health unit in Cumberland County, New Jersey.
- Must have a valid Driver’s License.
Qualifications
- Completed training as a NJ Certified Peer Recovery Specialist.
- Must be survivors of/or have lived experience with HCV with strong interpersonal skills who have experience in peer support or community-facing roles are highly desirable.

Pay
The annual base salary range for this position is $55,000-$61,000, with placement within the range determined by candidate's qualifications, professional experience, internal equity, and other factors.
