Responsibilities
- Provides direct/indirect therapy services to outpatients who require or will benefit from therapy skills and interventions.
- Provides case management services to coordinate care with primary care doctors, other prescribing physicians, probation, parole, child/adult services, and other supporting services.
- Attends all clinic team meetings and company training.
- Communicates at a level that reflects above-average quality work.
- Maintain client records and other data as required by regulatory agencies promptly.
- Treatment Planning: Applies assessment, evaluation, and reassessment data to the formulation of treatment plans. Supports recovery principles by incorporating the individual's and their family's (when applicable) strengths, barriers, and needs into the plan components. Documents treatment goals/objectives/discharge plans that are written in clear and understandable language and include clients' own words. Formulate measurable, achievable goals and objectives within the scope of treatment provided in collaboration with the client. Describes clinical modalities/interventions that are responsible and provide according to applicable protocols. Reassess treatment effectiveness and adapt treatment plan as needed.
- Maintains continuity among clinical teams by documenting and communicating actions, irregularities, and continuing needs.
Requirements
- Must Be Have CAT/CAS; or LAC/LCSW/LPC
- Have a Masters in Social Work/Behavioral Health
Qualifications
- One of CAT/CAS/Master/License (Required)
- Addiction counseling: 2 years (Preferred)
