About the role

Continuum Rehab Group offers in-house therapy services within Skilled Nursing facilities in Texas, New Mexico, Arizona, Indiana, and Missouri. Responsibilities

  • Utilizes evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with physical functioning disorders under the supervision of an Occupational Therapist.
  • Assesses and documents patient's progress and communicates with supervising Occupational Therapist to adjust treatments accordingly.
  • Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
  • Provides input to supervisor for discharge summary of services provided according to regulatory and clinical requirements.

Qualifications

This position is open to therapists at all experience levels. Required qualifications include:

  • Licensed Occupational Therapy Assistant.
