About the role

Flagstar Rehab COTAs work with a diverse and multi-disciplinary team to perform treatments and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives. Flagstar Rehab offers the opportunity for Direct Employment with our agency or to Be Your Own Boss as an Independent Contractor. We can assist you with setting up a corporation free of charge.

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress with the use of innovative EMR software. Proficiency in Rehab Optima + Point Click a plus!

Requirements

• Current NYS OTA License and Registration
• EMR Competence
• State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
• Valid Proof of Identity and Work Authorization
• Malpractice/Liability Insurance (if applicable)

Pay

Hourly Pay Rate: $34-$36 (W-2), $38-$40 (1099)
