Certified Occupational Therapy Assistant

Phoenix of Fairlawn is looking for a dependable and enthusiastic PRN Certified Occupational Therapy Assistant (COTA) to join our in-house rehabilitation team in a skilled nursing and long-term care setting.

Responsibilities

• Provide skilled occupational therapy treatments under the direction of an Occupational Therapist
• Work with SNF/LTC residents to improve independence, function, and quality of life
• Carry out individualized treatment plans and document resident progress
• Collaborate with OT, PT, SLP, nursing, and other members of the interdisciplinary team
• Complete timely and accurate documentation using Net Health
• Provide reliable weekend coverage for the rehab department

Schedule

• Every other Saturday availability needed
• Additional weekend coverage opportunities available
• Potential for additional PRN hours based on facility needs and availability

Requirements

• Current Ohio COTA license
• Graduate of an accredited Occupational Therapy Assistant program
• SNF/LTC experience preferred
• Familiarity with Net Health is a plus
• Strong communication and teamwork skills
• Dependable and comfortable working independently when needed
