Position Overview
Heritage Grove Estates is seeking motivated and compassionate Certified Occupational Therapist Assistants (COTAs) to join our in-house rehabilitation team for Full-Time and PRN opportunities.
Key Responsibilities
- Provide occupational therapy services under the direction and supervision of a licensed Occupational Therapist.
- Implement individualized treatment plans designed to support resident goals and maximize functional independence.
- Auxiliary residents with activities of daily living (ADLs), including dressing, bathing, grooming, mobility, and adaptive techniques.
- Monitor resident progress and communicate updates or changes in condition to the supervising Occupational Therapist.
- Accurately document treatments, progress notes, and outcomes in accordance with facility policies and regulatory standards.
- Collaborate with nursing staff, therapy team members, physicians, and interdisciplinary departments to ensure coordinated care.
- Inform residents and caregivers regarding therapeutic exercises, adaptive equipment, and safety strategies.
- Maintain a clean, safe, and organized treatment environment.
- Ensure compliance with ethical standards, facility policies, state regulations, and infection control procedures.
- Sustain a positive rehabilitation experience focused on resident-centered care and successful outcomes.
Qualifications
- Current Certified Occupational Therapist Assistant (COTA) certification and active state licensure required.
- Previous skilled nursing facility (SNF) or long-term care experience preferred.
- Strong communication and interpersonal skills.
- Ability to work collaboratively within an interdisciplinary team environment.
- Strong organizational and documentation skills.
- Commitment to delivering compassionate, high-quality resident care.
- Ability to adapt to changing resident needs and clinical priorities.
