Certified Nursing Assistant (PACE Home Care) - PRN

About the role

The PACE Home Care CNA plays a vital role in supporting the health and well-being of participants enrolled in the Program of All-Inclusive Care for the Elderly (PACE).

Responsibilities

• Assisting with bathing, dressing, toileting, meal preparation and feeding
• Monitoring vital signs and documenting intake and output
• Maintaining a clean, safe, and supportive environment through routine housekeeping tasks and chore services
• Transporting and assisting participants with simple errands

Requirements

• At least 18 years of age and has a High School Diploma or General Education Degree (GED)
• Currently a State Registered Nurse Aide through the Kentucky Board of Nursing
• Proof of completion of recognized CNA training program
• At least one year of experience working with the frail and elderly or proper training upon hire
• Food handling permit, to be completed upon hire
• Possesses and maintains current CPR certification
• Self-directed with the ability to work with little direct supervision
• Demonstrated knowledge and skills necessary to provide care to and communicate with the adult population
• Ability to read and interpret documents, write routine reports and correspondence, and collaborate with PACE staff and members of the Interdisciplinary Team
• Ability to speak effectively before groups of participants or team members
• Ability to apply common sense understanding to carry out instructions, deal with problems, and follow emergency and evacuation procedures
• Ability to accommodate schedule changes daily due to staffing, participant condition and needs, and new participants
• Licensed driver with automobile in good working order, insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage
• Demonstrates tact, patience, and good personal hygiene
